Please write a complete Google Apps Script that automatically creates a Google Form for collecting leads from parents interested in my tutoring or coaching programme.
The form should collect the following fields:
1. Parent's name (short answer, required)
2. Child's education level (multiple choice — required)
Options: Primary 1, Primary 2, Primary 3, Primary 4, Primary 5, Primary 6, Secondary 1, Secondary 2, Secondary 3, Secondary 4, Secondary 5, JC 1, JC 2, Other
3. Contact number (short answer, required)
4. Email address (short answer, required)
5. What is the biggest challenge your child is facing right now? (paragraph, required)
Script requirements:
- Create the entire form programmatically using Google Apps Script
- Set the form title to: "Tell Me About Your Child"
- Set the form description to: "I'd love to learn more about your child and how I can help. This takes about 2 minutes to fill in."
- Make all questions required
- Set the confirmation message (shown after submission) to: "Thank you! I'll be in touch within 24 hours."
- Link the form to a new Google Sheet to collect responses automatically
- Name the Google Sheet: "Leads"
After the script, please also give me:
